I had a patient who was taking warfarin for their anticoagulation therapy and happened to miss some doses due to a family matter. Upon checking her INR, it was in the low 1's, her goal was 2.5–3.5, and needed to be bridged with enoxaparin. I sent a prescription to the pharmacy, but her copay was over $100!!
